What Information We Collect and How We Use That Information:
Our website’s optional log-in and sign up forms may require users to select a user name and password. The forms also require that users give us contact information that may include name, e-mail address, format preference (HTML vs. Text), address, interests, and similar information. We do not request or store sensitive information from our visitors, such as credit card or social security numbers. Users who sign up have the option at any time of opting out and will receive no further communications from DNA.
Sign Up to Receive E-mail News:
Donor Network of Arizona may invite website visitors to sign up to receive e-mail news. Initial information requested will include first name, last name and e-mail address. Because participation is optional, visitors may choose to submit their information or skip this step. If they do opt-in, they will be entitled to opt-out at any time. DNA stores this information for the sole purpose of sending e-mail news. DNA may send e-mails to subscribers seeking additional demographic and other information. Visitors will have the option of responding or not to such requests.
Donor Network of Arizona’s online surveys request information that includes: First and last names, e-mail address, and check-off boxes for questions such as whether the visitor would like to receive e-mails and/or postal mail from the Donor Network of Arizona. Because participation in online surveys is optional, visitors may choose to submit the surveys or skip this step.
If a user elects to use our referral service for informing a friend about one of our Sites, we ask for the friend’s name and e-mail address. Donor Network of Arizona will automatically send the friend a one-time e-mail inviting them to visit the site. DNA stores this information for the sole purpose of sending this one-time e-mail.
Enrollment in the DonateLifeAZ Registry:
By enrolling in the DonateLifeAZ Registry, individuals 15 years 7 months of age and over give legal authorization for the recovery of their organs, tissues and eyes for the purposes of transplantation and/or research at the time of their death, as indicated by the anatomical gifts they choose during the registration process. Registry information is kept strictly confidential and can only be accessed by Donor Network of Arizona. Such access can only be for the purpose of identifying potential organ and tissue donors at or near the time of death.
Internet Protocol Address:
We collect an IP address from all visitors to our Sites. An IP address is a number that is automatically assigned to your computer when you use the Internet. We use IP addresses to help diagnose problems with our server, administer our Sites, analyze trends, track users’ movement, gather broad demographic information for aggregate use in order for us to improve the site, and deliver customized, personalized content. IP addresses are not linked to personally identifiable information.
Use of “Cookies”:
All information provided to Donor Network of Arizona is transmitted using SSL (Secure Socket Layer) encryption. SSL is a proven coding system that lets your browser automatically encrypt, or scramble, data before you send it to us. We also protect account information by placing it on a secure portion of our Sites that is only accessible by certain qualified employees of Donor Network of Arizona. Unfortunately, however, no data transmission over the Internet is 100% secure. While we strive to protect your information, we cannot ensure or warrant the security of such information. Notwithstanding anything contained herein to the contrary, DNA makes no representations regarding the privacy of any information that you send to us while it is still on route from you to us, whether it be by mail, phone, fax, e-mail, Internet, or any other method of communication.
